Yes, it comes just like shown in the picture.

Is the grease for the synthetic gears as well as the metal gear?

If I remember correctly the grease was for the synthetic gears. I would do a Google search for part # 41C4220A. That's were I found a link to a YouTube video that demonstrated how to repair your garage door motor using this kit.

From 1 through 3 how easy is it to replace those parts from the original unit, 3 means you need to hire a contractor.

If you have any mechanical ability and can follow written directions, I would rate it an easy 1. You can also get help on line from several DIY web sites. Just don't tighten the chain/cord too tight. It needs to sag about 1/2 to 5/8 inch, in my opinion.
