Not truly isolated. The input and output grounds are connected together.

Sort of works
Ok, this does work but not well, well enough for my needs though. This module does power up and work on 6v but obviously you'll then only be able to use the 0v-5v output setting. On that note on the module I received, using a 24v power supply, with the PWM at 100% duty I could only get 3.9v on the 0v-5v setting and 7.6v on the 0v-10v setting. This means that you won't be able to get full RPM from your motor, mine was down by about 20% so I have used the 0v-10v setting to supply the 0v-5v input on my motor driver trimming down the output voltage to as low as it would go (around 6v). As I said it works for my application. I did try a 30v supply but it made no difference.
