Is this the actual soap or lotion?

It is very much like soft soap, but for your hands. There is pumice or grit material mixed in with the cleaner.

How does this "extreme" version compare with the old 25219 fast orange pumice lotion hand cleaner?

If used correctly. I haven't found anything it won't take off. I use mine for removing dried on automotive paint. Put it on, scrub vigorously, and rinse away the grime. To answer your question directly. It has more abrasive.
