The nose piece can be rotated in an effort to reverse the flow of saline water. I heat distilled water for 30 seconds in the microwave prior to adding the solution to the supplied saline cartridge. I am always amazed at how much garbage is taken away from the process. I get nice relief for the start of my day. I also use the unit at the end of the day which provides me with nighttime relief so that I can sleep. I think that this is an awesome product and is superior to the other units which only allows for saline to be inserted in only one nostril without the ability to remove the saline. There were no issues with it, just not compatible with my body.
