I got this after brachioplasty (arm lift) surgery. It stays well enough in place without the removable strap at the top. The sleeves don't roll up. It's excellent quality. I do recommend sizing up from what you'd normally wear. It runs small. And while you want that nice tight pressure around the arms, if it's too small it is suffocating.
