In my case, I wear a catheter while recovering from surgery and have been advised to keep the catheter "capped" during the day to give the bladder exercise. Others mention that the size is smaller than advertised. I would add that the advertised size is WAY larger than it should be. I have only a 34" waist which would make me eligible for a medium, but not only was the medium too tight, the large was also uncomfortably tight.
