They should cut out or remold the leather to form fit around the rear sight better, although in time the leather will form around the sight. I put the 586 away and carried my Ruger GP-100 3" fixed sight in the holster... PERFECT!!! Metal clip was a bit thin, but not a big deal, easily replaced. For a 4" or 3" S&W 581/681, or Ruger GP-100 with fixed sights, this holster is comfortable, rugged and gives a snug, secure fit for IWB carry.
