I have a medium size face and am female, so I usually buy size M masks. After reading the reviews that the masks run a bit small, I decided to buy the large size. Glad it did. I think the smaller size would have fit, but would have been pressing against my face and mouth too much. With the large size, it hangs down more freely with more airflow. When I don't need it, I can pull it down around my neck and it's not tight. The elastic strap is enough to keep it in place while I'm walking, without being tight.
